Sedar x sedar, ana da pon dlm final yer...wahh, kejap jer rasernyer...tetiap ari ade clinic (kejap POC, kejap CL), tetiap ari pk sal proposal research, proposal ENT, proposal health promotion, log book...huhu...pnat n penat n penat lagi..tp best, tetiap hari ader byk bnda bleh blajar...

case ari ni:

50 y.o/F/ M

comes with no sx (not blur with cx spectacle, @ distance and near, not sign of BV prob)

cx spectacle;3 y.o (OD: -1.00/-0.25 x 70 (6/6 -2), OS: -1.25/-0.50 x 76 (6/9+2)), Add: (+1.75)

have allergies, mild sinusitis, no general health problem reported

Refraction:

Rx: OD: -1.00/-0.50 x 75 (6/6 -1)

OS: -1.25/-0.50 x 95 (6/6-2)

Add : +2.25

*pt report dat the letter is small (for near and distance) and if the +0.25 was put, the vision is not clear.

*through retinoscopy, the cataract can be seen at the center and it already mature

All the entrance test can be done, and it is normal for her age.

IOP (Keeler pullsair):

OD :20mmhg

OS : 21mmhg

Slit Lamp Examination:

OD : papillae grade 2 on upper palpebral, concretion at lower palpebral, pingucuella at nasal and temporal side, superficial staining grade 1 at 7 o'clock and 12 o'clock, TBUT(4-5 sec), lens opacities at posterior part, grade 3 angle depth

OS : papillae grade 2 at upper palpebral, pingucuella at temporal and nasal side, cornea is clear, TBUT (2-3 sec), lens opacities at posterior part, grade 3 angle depth

Direct Ophthalmoscope:

OD: 0.4 CD ratio, 2/3 AV ratio, dull reflex and not full

OS: 0.4 CD ratio, 2/3 AV ratio, dull reflex and not ful

*examiner has to tilt from upper and temporal side.

Finding:

BE : Low Myope with low ATR astig, Dry eye, pingcuella, dense cataract, slightly high IOP, papillae grade 2

OD: Concretion and superficial staining grade 1

Management:

Not prescribe new Rx as the rx is about da same and she does not have any complaint about vision.

Patient refer to hospital for further examination on cataract evaluation.

Advice for warm compression to reduce papillae.
